Referral and Admission
FTS Care understands that when a young person moves into a new home environment, that process can prove to be stressful and challenging both for the young person and for their family.
FTS’s admission process has been designed to make moving in to the new home environment both a positive and stress-free experience for the young person and their family and that allows for a thorough and comprehensive transitional and admissions process to take place.
Our admission procedure is the following...
- An initial contact between ourselves and the placing social worker will take place to discuss the young person, their needs and requirements and what we can offer to meet those needs.
- To request, where appropriate, and to review any paperwork and documentation pertaining to the young person.
- If both we and the placing social worker feel that FTS Care can support the young person and meet their needs then we would arrange for the placing social worker to visit our home, inspect it, and ask any questions and to satisfy themselves of the suitability of our service.
- A further review by us of appropriate paperwork including “LAC” forms, Psychologist’s report, chronology of the young person’s life and reviews of behaviour.
- FTS Care will, at this point, give an indication of fee costs.
- Should both FTS Care and the placing social worker feel that they both wish to progress with placing the young person, then a meeting with the young person will be arranged to discuss the move, to understand the young person ‘s requirements and needs and to talk over any issues or uncertainties the young person may have about moving in.
- A risk assessment will take place after the meeting and appropriate outcomes agreed with social services.
- FTS Care will confirm their fee structure for placing the young person, contract documentation put in place and signed and an entry date set for the young person.
- Where possible, a pre-entry visit by the young person and their parents / Carers will be arranged to get to see the home, its facilities and to speak to and get to know the staff.
